Candidates who choose to download the form must send a Demand Draft of Rs. 1000/- (Rupees one thousand only) in favour of The Director, SDM Institute for Management Development payable at Mysore.

- Affix a passport size photograph at the appropriate place in the Application Form.
- Enclose Demand Draft (Rs.1000) / photocopy of CAT/XAT admit card, coaching centre receipt (if issued). If downloaded send the filled application along with photocopy of CAT/XAT admit card, Demand Draft/Multi City Cheque/E-Payment challen or proof of E-Payment.
